USCIS exam. FIRST VISIT

Service Description

This is the FIRST visit for the USCIS exam (it usually takes two visits) You will not need this visit if you pay for the labwork in advance. we will need your name, date of birth and address and then we can collect the fee in advance via zelle. Zelle email is floridaflightdoc@gmail.com. To prepare for your visit, please send us your 1) vaccination records, 2) driver's license or passport, and 3) submit your immigration physical form. You can find the immigration physical form in the tabs above. you can email us questions at floridaflightdoc@att.net. Vaccinations required for an appointment are the following: Covid, MMR, tetanus, flu, varicella, and Hepatitis B. Patients older than 65 will also need the pneumonia vaccine. Patients born before 1957 will not need MMR. If you are a family of 2 or more, only ONE person needs to come to the appointment but must bring all the people's information as listed above.
