This is the medical exam after you have done all the laboratory tests and vaccines. If there are more than two people in your group, then make TWO appointments on the system. To prepare for your visit, please bring your 1) vaccination records, 2) driver's license or passport, and 3) submit your immigration physical form. You can find the immigration physical form in the tabs above. 4. ALWAYS bring your vaccine records and your covid card, even if you have already sent them to us, ALWAY bring your ID even if you have already sent them to us.
